Transmission

Cholera spreads through food and water contaminated with Vibrio cholerae bacteria – typically in regions with poor sanitation or active outbreaks. Risk is highest with unfiltered water, ice, undercooked seafood and street food.

Geography

Endemic across sub-Saharan Africa, parts of South Asia, Yemen, Haiti and Latin America, with periodic outbreaks elsewhere. NaTHNaC recommends Dukoral for travellers to high-risk regions, aid workers and visitors with limited access to safe water.

Immunity & dosing

Dukoral is taken as two oral doses 1โ€“6 weeks apart, giving around 85% protection that lasts ~2 years in adults (6 months in children 2โ€“6, who need a third dose). It also provides short-term protection against ETEC (traveller’s diarrhoea).

If you develop these within 1โ€“3 weeks of return from an endemic region, contact NHS 111 – mention recent travel.

Watery diarrhoea

Rice-water stools, very high volume.

Vomiting

Often forceful, contributing to fluid loss.

Rapid dehydration

Within hours in severe cases.

Muscle cramps

Calves, arms - from electrolyte loss.

Sunken eyes / shock

Severe dehydration sign - emergency.

Reduced urine output

Low or no urine - kidneys struggling.

Low blood pressure

Light-headed, fainting - call 999.

You should book cholera vaccine if…

Not suitable for children under 2 years (Vi vaccine) – oral Dukoral is licensed from age 5. Severe allergy to a previous dose excludes. Pregnancy: weigh up exposure risk with pharmacist.

Ideally at least 7 days after dose 2 for full protection. Doses are taken 1โ€“6 weeks apart, so plan 2โ€“6 weeks ahead. Last-minute travellers still benefit from partial protection.

Yes – every 2 years for adults if you continue travelling to high-risk regions. Children aged 2โ€“6 need a booster every 6 months.

Children under 2 years, people with severe allergy to a previous dose or vaccine components, and certain acute gastrointestinal illnesses. Pregnancy: discuss with our pharmacist on the day.

Most people get mild stomach upset, nausea or loose stools for 24โ€“48 hours. Serious reactions are very rare. Drink plenty of water for 1 hour either side of each dose.
